Requests

Network Resource Access is managed IS and authorized by the departments that own the Resource. In order to gain access to Network Resources, access forms are completed and sent to FMIS requesting the specific resources you are to have access to.

We've provided links on this page to get you to the information and forms most frequently requested.

Forms can be delivered to USIT at 300 Donhowe Building or set as an attachment to an e-mail to (usitss@umn.edu).

To request access to the Image Site Document Repository, download the Project Access form below. This form gets emailed back to () and your ID and password are emailed back to you. Information on how to use Image Site can be downloaded from the link below.
